East Valley Basketball Service Agreement

1. Introduction This Service Agreement ("Agreement") is entered into by and between East Valley Basketball ("League," "we," "us," or "our") and the participants, parents, guardians, and coaches ("you" or "your") who engage in our services. By participating in the League, you agree to the terms outlined in this Agreement.
2. Services Provided East Valley Basketball offers youth basketball programs, including but not limited to team practices, games, tournaments, and related activities ("Services"). These Services are designed to promote physical activity, teamwork, and sportsmanship in a supportive environment.
3. Registration and Fees
- Registration: To participate in the League, you must complete the registration process, including providing accurate and up-to-date information.
- Fees: Participation fees are required for enrollment in the League. Fees cover coaching, facility use, equipment, and other related expenses. All fees are non-refundable unless otherwise stated in writing.
4. Code of Conduct All participants, parents, and coaches are expected to adhere to a code of conduct that promotes respect, integrity, and sportsmanship. Any behavior that is disruptive, harmful, or unsportsmanlike may result in disciplinary action, including suspension or expulsion from the League.
5. Health and Safety
- Medical Clearance: Participants must obtain any necessary medical clearance prior to engaging in physical activities. East Valley Basketball is not responsible for any injuries or health issues that arise during participation.
- Safety Protocols: We implement safety protocols to ensure the well-being of all participants. Compliance with these protocols is mandatory.
6. Liability Waiver By participating in the League, you acknowledge and assume all risks associated with physical activities, including potential injuries. East Valley Basketball is not liable for any injuries, losses, or damages that occur during participation.
7. Privacy and Data Use
- Personal Information: We collect and use personal information as outlined in our Privacy Policy. This information is used to facilitate your participation in the League and improve our services.
- Data Protection: We are committed to protecting your personal information and ensuring that it is handled securely.
8. Use of Likeness By participating in the League, you grant East Valley Basketball the right to use photos, videos, and other media of participants for promotional purposes, including on our website, social media, and marketing materials.
9. Termination East Valley Basketball reserves the right to terminate this Agreement and your participation in the League at any time for violations of this Agreement, including non-payment of fees or breaches of the Code of Conduct.
10. Changes to Services We reserve the right to modify, suspend, or discontinue any aspect of the Services at our discretion. Participants will be notified of any significant changes.
11. Governing Law This Agreement shall be governed by and construed in accordance with the laws of the state in which East Valley Basketball operates.
